DOT Physical Medical Question

    I am wondering if I am able to give a CDL med card to someone who has permanent restrictions from a work comp injury (no lifting over 90 pounds from floor-to-waist and no overhead lifting over 50 pounds)? I have had no luck reaching out to the FMCSA or my state office for guidance and I work in a small practice and no other providers here have run into this. Thanks in advance!

    Great question! Sorry for the delay this one stumped me too. I looked and couldn’t found a pound lifting requirement anywhere. I’ve always been told not to pass with any kind of restrictions. I did ask Dr Hartenbaum to weigh in and this is her response:

     The answer is complicated. There is no ONE specific number but a lot of resources examiner should use to evaluate whether the individual can perform ALL potentially required duties of a CMV operator.  MEs must remember that a certification is good for ANY CMV operations/ company and the ME cannot place any restrictions aside from corrective lenses/s hearing aid or SPE or other variance (exemption or certification under an alternative standards

    Sources MEs should consider.

    The CMV driver demands and duties found in the ME handbook not once, but twice.  Section 3.3 and Section 4.9.3.1 

    The Introduction to the Medical Examination report form is another good portion to consider.

    Truck Driver - heavy (remember the certificate is good whether the individual drivers a bakery truck weighing 11K pounds but the bread crosses state lines or a vehicle weighing well over 26 K and being required to load and unload.) is generally considered a medium to heavy job.”

    Thanks for your response, Nicolette! This is very helpful. I have Dr. Hartenbaum's guide to the DOT Medical Examination at my ready always, such a great resource. Does she share contact information for complicated medical questions that come up? I am still trying to find good resources for DOT medical examinations. 

    FYI I did hear back from the FMCSA Medical Programs Division so wanted to share that information in case you run into it in the future: "As far as certifying a driver who has a workers comp lifting restriction, you can qualify the driver as the medical examiner as long as you are confident in knowing the lifting requirements of the driver and that the restriction doesn't interfere. You would document everything on the examination and you can issue a certificate. What you cannot do is issue a restricted medical examiner certificate."
